This letter is in response to your recent complaint of unsafe and / or unhealthful conditions at, the above referenced establishment. The information provided has been inserted onto the enclosed Notice of Alleged Safety or Health Hazards, OSHA-7 Form. Please sign on the “signature” line, Item 16, and complete Items 17 and 18, if applicable. Also, provide your job title with this company after your signature.
Please note that information is needed for those items marked with an asterisk (*). If you are aware of new or additional information you deem important, please submit that information on a separate sheet. It is imperative that you refrain from marking on the enclosed OSHA-7 Form as doing so will delay the processing of your complaint.
Please mail the signed Notice of Alleged Safety of Health Hazards, OSHA-7 Form, in the self-addressed, stamped envelope provides for your use. This letter must be postmarked no later than March 3, 1989. If you do not mail the signed form by March 3, 1989, your complaint will be treated as a nonformal complaint
Your interest in workplace health and safety and your cooperation is appreciated. If the conditions you describe provide reasonable grounds to believe that a violation threatening physical harm exists, an inspection of your workplace will be scheduled after we receive the signed complaint form.
